We have an exciting opportunity for an experienced Clinical Care Team Leader to join our team based in Camberwell. Reporting to the State Manager, you'll be responsible for leading the Victorian Care Management team. Your experience and thoughtful decision-making will support Care Managers to ensure person-centred, individualised care plans are in place for all clients, and clinical care needs are identified and managed.   

This is a 1-year Maternity Leave Contract.

Key Responsibilities:

• Supervision and mentoring of the Care Management team 
• Conducting clinical assessments 
• Evaluating clinical interventions to ensure the safe and effective provision of care.
• Managing service agreements with external providers as required to contribute to the assessment and in-home clinical intervention processes, working in collaboration with other health professionals and ensuring compliance with all relevant legislation, guidelines, and best practices
• Apply ethical standards of practice and use professional knowledge and experience to provide advice on a range of issues involving clients with complex care needs
• Participate in the resolution of stakeholder feedback, including compliments, hazards, incidents and complaints. 
  
  
• What you'll bring to the role:
• Tertiary qualification in Nursing and current AHPRA registration  
• Proven experience in Leadership roles
• Ideally, you will have familiarity with Client Management Systems software.
• Understanding of the aged care and disability sectors, including regulations and compliance
• Full Working rights in Australia

 To work in our Home care, you will need to provide satisfactory background checks, supply an NDIS worker screening clearance and proof of an annual flu vaccination
