The Opportunity:

As a Regis employee, you’ll be part of our BodeWell Community Care team – working together to enhance the wellbeing of older Australians. BodeWell is an approved provider with the Department of Health, delivering government-funded Home Care Packages, Private Aged Care, and NDIS services across Queensland and Victoria.

You will also deliver specialized clinical services tailored to the unique needs of each client within a home care setting. This role ensures that our participants are supported to live independently in their home by linking them with Directed Care Supports; engage in the development of assessment and goal planning, create referral’s to appropriate services, ongoing monitoring of Budgets and participation in plan reviews, while following Operational Guidelines and NDIS legislation and procedures.
 Key Responsibilities:

  • Working collaboratively with participants to foster capacity building and independence, aiming to facilitate the achievement of participants' goals as outlined in their NDIS plan
  • Undertake assessment, care planning and care coordination activities required to meet the diverse needs of the Participants
  • liaison between participants and service providers, ensuring that the most suitable services are engaged to meet individual needs, 
  • Conducts home visits with participants to complete necessary documentation, including assessments, home environmental checklists, support plans, and service agreements.

 What you’ll bring to the role:

  • Masters or Advanced Diploma in Counselling/Mental Health/ Social work or similar for Level 3 specialist support coordination or 
  • Bachelor in Nursing, Allied Health or Social Work / Welfare or Human Services
  • NDIS Clearance or a willingness to obtain 
  • Experienced in assessment and record keeping via databases and or client management systems, ie
  • Experience working in the aged care and or the disability field, ideally as a Care Manager / Support Coordinator
  • A valid Driver’s License and reliable vehicle
  • Previous experience in NDIS or relevant programs is required
  • Demonstrated experience in managing client budgets

 To work within Home Care, you will need to provide satisfactory background checks, supply an NDIS worker screening clearance, evidence of current flu vaccinations.

The important role you will play


Key responsibilities include:


  • Facilitation of learning solutions across clinical, carer, operational and non operational and management subject matter areas 
  • Cyclical learning needs analysis across workforce segments 
  • Creation of annual calendar and scheduling of core capability programs with key operational areas
  • Evaluation and continuous improvement through implementation of Learning Transfer Evaluation Models to assess impact and effectiveness
  • Stakeholder engagement
  • Implement learning champions design to embed learning into the flow of work


What you will bring to the role  


  • A diploma or higher-level qualification in adult education (or related discipline) and/or a Cert IV in Workplace Training & Assessment is required
  • Registered Nurse qualifications are highly desirable
  • NDIS worker checks are required
  • Mastery in facilitation of learning across multiple modalities (in person, in the flow of work, on digital platforms) with various learner audiences that achieve business results.  Presentation and communication skills are critical to deliver content effectively 
  • Demonstrated experience in delivering competency-based learning, systems-based learning, management capability learning and team dynamics workshops
  • Demonstrated experience in creating a learning environment that is safe, stimulates learning, growth and learning transfer
  • Depth of experience in working with various learning artefacts (session plans, participant guides, facilitator guides, performance support and implementing them through learning delivery 
  • Demonstrated experience in applying learning technologies  
  • Expertise in working with MS Office 365, knowledge repositories and learning management systems
  • Strong ability to manage group dynamics, facilitating discussions, managing diversity of perspectives, de-escalating conflict and providing learner feedback 
  • Ability to monitor and tracker learners’ performance during the learning solution being delivered, providing constructive feedback and adjusting as required to meet the learning objectives 
  • Demonstrated preparedness, organisational planning and time management skills 
  • Facilitators/trainers demonstrate currency with the latest learning techniques, pedagogical advancements, and industry trends
  • Critical thinking to analyse situations and make sound decisions
  • Customer service experience in providing an effective and efficient learning operations, responding and resolving enquiries
